LEAVE A GIFT IN YOUR WILL, TRUST, OR BY BENEFICIARY DESIGNATION

To leave a gift to the CACNA1A Foundation in your will, trust or by beneficiary designation, please use the following language: “I hereby give and bequeath $_________, or _____ percent of the residue of my estate to the CACNA1A Foundation, Inc., a nonprofit located in Connecticut and exempt from Federal taxation under Section 501(c)(3) of the Internal Revenue Code, Federal tax ID number 84-4985747.”
